Procedures
Please review the court’s Administrative Order FLMB 2023-3, which describes procedures for how a Chapter 13 Bankruptcy works in our Bankruptcy Court, for information on how your case should proceed. Paragraph 3 of this Order refers to a “Model Plan”, which is required for all Chapter 13 Bankruptcy cases in the Middle District of Florida.
Model Chapter 13 Plan
If you have already filed a different version of a Chapter 13 Plan, please re-file your plan using the Model Plan format required by the Court.
§ 341 Meeting of Creditors
Your Official Form 309I - Notice of Chapter 13 Bankruptcy Case lists the date and time for your § 341 Meeting of Creditors held virtually via Zoom.
In advance of your § 341 Meeting of Creditors, you must provide the following documents to the Trustee no later than seven (7) days prior to your § 341 Meeting of Creditors date:
- A clearly legible unredacted color photo of your Florida Driver’s License;
- A clearly legible unredacted photo of your social security card. If you do not have your social security card readily available, you may provide another document prepared by a 3rd party that confirms your full social security number. One common example of this would be a W-2 issued by an employer.
- Copies of your Income Tax Returns for the three (3) years prior to filing your Bankruptcy case. Income tax returns must be redacted. Please cover the names of all dependents, your social security number except for the last 4 digits, and any bank account information that might appear for the purposes of a refund.
- Copies of 3 months of bank statements for the 3 months before filing. Please redact your bank account number except for the last 4 digits. If you are self-employed, we will need both personal and business bank statements.
- Copies of 3 months of paystubs, or, if you are self-employed, 6 months of profit and loss statements.
- Completed and signed 341 Disclosure Forms.
There are two ways for you to provide the above-referenced information. You can scan and upload it to the Trustee’s Business Records and Taxes (“BRAT”) portal or mail everything to the following address:

Due to the sensitive nature of the information, the documents cannot be emailed or faxed.
